Start with the issuing authority
A replacement request normally follows the authority that issued or holds the original record. Dubai-issued records may use the DHA or Dubai Health route, while records handled through federal services may use Emirates Health Services.
Review the DHA issue/copy/lost-replacement service and the EHS lost-certificate replacement service.
Information to prepare
- Where and when the certificate was issued.
- A copy or reference details, if available.
- Identity evidence requested for the certificate holder and parents or authorized representative.
- Police or authorization evidence when the official service requires it for the circumstances.
DHA’s public guidance indicates that a police letter may be requested when a copy of a lost certificate is unavailable. Verify that requirement for the exact case before obtaining documents.
