1. Identify the record holder
Start with the authority or health system that issued or retains the birth record. Do not begin an unrelated new-birth application.
2. Check the official replacement service
Review the DHA issue/copy/lost-replacement page and the EHS replacement service.
3. Prepare identity and record evidence
Gather the identity evidence requested for the certificate holder, parents or authorized representative. If you have a copy, notification/reference number or prior correspondence, keep it available.
4. Check special evidence requirements
DHA’s public information indicates that a police letter may be requested if a copy of the lost certificate is unavailable. Outside-UAE applicants may also need authorization evidence. Verify the current requirement for the case.
5. Apply and verify the new copy
Use the official channel, pay the current authority fee and review the replacement for consistency before using it in another process.
